THE COMMONWEALTH OF KENTUCKY
WHEREAS, Samuel Prickett, has been appointed by the County Court of Campbell County, and has qualified as Guardian to Michael Burkman, a minor.
NOW, we Samuel Prickett as principal, and David Burkman and Jacob Phillips, his sureties, do hereby covenant to and with the Commonwealth of Kentucky, that the said Samuel Prickett, will faithfully discharge the trust of Guardian to said minor, in all respects as required by law.
