THE COMMONWEALTH OF KENTUCKY
WHEREAS, David Mayall, has been appointed by the County Court of Campbell County, and has qualified as Guardian to Thomas M Stevens, a minor child of Jacob Stevens.
NOW, we David Mayall as principal, and Jno S Youtsey, his sureties, do hereby covenant to and with the Commonwealth of Kentucky, that the said David Mayall, will faithfully discharge the trust of Guardian to said minor, in all respects as required by law.
