The fee for the certificate is $6.00 and the request must include the full name, exact year of death, and one of the following: the month and day of death, county or city of death, or death certificate number. If you do not have enough information to request a certificate, you may send a research request. The fee for this request is $20 and two indexes will be searched for one name.
